Description and Use: The Federal Grants and Cooperative Agreement Act of 1977 requires Federal agencies to use some type of grant agreement as the legal instrument to transfer money, property, or services to state or local governments. The U.S. Fish and Wildlife Service uses these forms to make or amend awards for approved grant projects. This includes determining if the estimated cost is reasonable, the cost sharing is consistent with the applicable program statutes, and whether sufficient federal funds are available for obligation. The state uses the information to request funds and identify proposed cost sharing. Federal cost distribution provides the State an opportunity to consolidate work under a single project for funding by more than one grant program and distribute the Federal share among applicable programs. The information collected on the amendment form is used to identify and approve proposed changes in the grant agreement.

Frequency of Collection: Generally annually.

Description of Respondents: State, Territorial (the Commonwealth of Puerto Rico, the District of Columbia, the Commonwealth of the Northern Mariana Islands, Guam, the Virgin Islands, and American Samoa), local governments, and certain others receiving grant funds.

Estimated Burden To Complete Form: One hour.
