Description: This data collection is necessary to fulfill the mandate of Executive Orders 13198 and 13199 dated January 29, 2001. These Executive Orders require the removal of barriers to the full participation of faith-based and community organizations in federal social service programs. This data collection will provide the data to determine the level of faith-based and community participation in the DOL grant programs. The data collected on this form is identical to that collected on OMB number 1890-0014 which has been approved by the Office of Management and Budget for use government-wide.Start Signature
